Cardiff University has issued an apology after a student took her own life when she was wrongly told that she had failed her exams.
Mared Foulkes, 21, was told that she could not progress to the third year of her university degree due to her exam results.
Foulkes was in the second year of her pharmaceuticals degree at Cardiff University when the Covid pandemic hit.
The 21-year-old, like other students across the country, had to continue her course online and through Zoom calls.
An inquest heard that an automated message from her university told Ms Foulkes that she had failed her recent exams and would not be moving onto her third year.
The email arrived hours before Foulkes took her own life.
However, her result was later corrected by the university to give her a pass. Tragically, Ms Foulkes, of Cae Uchaf Farm, Menai Bridge, Anglesey, had already taken her own life as the mistake was rectified.
After an inquest in October last year, the university confirmed it will simplify its exam process after a coroner said it was "confusing" and "complex".
The university has now apologised to Mared's family and said there will be changes to how exam results are communicated.
Vice Chancellor of Cardiff University Prof Colin Riordan admitted the previous process had "room for confusion".
He said: "Exam results will be communicated in a way which leaves no room for confusion in the future. I apologise, absolutely.
"I'm deeply sorry for this sequence of events. It's a devastating set of circumstances and I absolutely understand the family's need for answers to their questions.
"I'm very happy of course to speak to Mared's parents, if that's what they would like".
